What to Bring to Your First Visit

As you plan for your first visit with your physician, here are a few items you should remember.

• Insurance Information: Please bring your insurance card. If your insurance provider requires a referral, bring referral number and forms.
• Medical Records: Please have your medical records from previous physician transferred prior to your appointment.
• Medications: Please bring a list and/or all the medication bottles of all medications you are currently or recently taken, whether over the counter or prescription. Include the strength and dosage of each medication.
• Patient Information Form: If you did not receive a new patient information form in the mail, please download and print the form below and bring completed form with you to your appointment.
